Responsibilities include:

- Providing extensive support in the shaping of individual budgets for disabled children and their families
- Developing and modelling individual client pathways through the NHS
- Undertaking specific projects to examine the health element of clients? individual budgets
- Supporting the implementation of project milestones and monitoring the delivery of outcomes
- Carrying out data collection and analysis which will be used to model costs for alternative provision
- Set up project groups and liaise closely with the client families

You should have a background of working within primary care and have experience in leading and implementing small projects in the NHS. You should also be able to demonstrate an interest in community health and social care for children, young people and families.
